Sometimes it can be difficult to know the proper way to fill out your W-9, here are instructions for your specific needs.
W-9 as a Sole Proprietor
- First, grab your W-9 form.
- Put your name on line one.
- Line two will be left blank.
- Check the individual/sole proprietor or single-member LLC
- One way to know if you are a sole proprietor is if you fill out a schedule C form when you're filling out your personal taxes.
- Fill out lines 5 and 6 with your business address.
- For Part 1, make sure you fill out either your social security number or your EIN, if you have one.
- Make sure to choose SSN or EIN, never both.
W-9 as a Sole Proprietor with a DBA
- First, grab your W-9 form.
- Put your name on line one.
- Fill out your business name on line two.
- On line three check the first box individual/sole proprietor or single-member LLC.
- One way to know if you are a sole proprietor is if you fill out a schedule C form when you're filling out your personal taxes.
- Fill out lines 5 and 6 with your business address.
- For Part 1, make sure you fill out either your social security number or your EIN, if you have one.
- Make sure to choose SSN or EIN, never both.
W-9 as a Single-Member LLC
- First, grab your W-9 form.
- Put your name on line one.
- Fill out your business name on line two.
- On line three check the first box individual/sole proprietor or single-member LLC.
- One way to know if you are a Single-Member LLC is if you fill out a schedule C form when you're filling out your personal taxes and have an EIN.
- Fill out lines 5 and 6 with your business address.
- For Part 1, make sure you fill out either your social security number or your EIN.
- It is more common to use your EIN if you are registered as an LLC.
- Make sure to choose EIN or SSN, never both.
W-9 as a Multi-Member LLC
- First, grab your W-9 form.
- Put your business name on line one.
- Line two will be left blank.
- On line three, check the box for Limited Liability Company.
- Multi-Member LLC are by default taxed as a partnership, so write in the letter P.
- Your business is a Multi-Member LLC if you fill out a 1065 form when you fill out your income taxes.
- Fill out lines 5 and 6 with your business address.
- For Part 1, make sure you fill out your EIN only.
W-9 as an LLC Taxed as an S Corporation
- First, grab your W-9 form.
- Put your business name on line one.
- Line two will be left blank.
- On line three, check the box for Limited Liability Company.
- You will then fill in the letter S, for an S Corporation.
- Your business is an LLC Taxed as an S Corporation if you fill out an 1120-S form when filling your taxes.
- Fill out lines 5 and 6 with your business address.
- For Part 1, make sure you fill out your EIN only.
W-9 as a C Corporation
- First, grab your W-9 form.
- Put your business name on line one.
- Line two will be left blank.
- On line three, check the box for C Corporation.
- Your business is an C Corporation if you fill out an 1120-C form when filling your taxes.
- Fill out lines 5 and 6 with your business address.
- For Part 1, make sure you fill out your EIN only.
